I give permission for my son/daughter to attend the event. I have received information regarding this event and I am aware that if I have any questions regarding it I can contact the group leader for this event (name): James Stevens, Assistant Pastor, on (phone number) 07469 822563.

I have provided the requested contact information for my child, his/her DOB, as well as any necessary emergency contact details for myself, and secondary contact should I not be reachable.

In addition, I have provided the name and contact number for my childs GP, along with any known conditions, allergies etc. (e.g. asthma, diabetes, epilepsy) and any medication being taken.

I have provided any other special needs, requirements or directions that would be helpful for the leaders to know about.

I will inform the leaders of any important changes to my child’s health, medication or needs and also of any changes to our address or to any of the phone numbers or details provided.

If I cannot be contacted and my child should require emergency hospital treatment, I authorise an adult leader to sign on my behalf any written form of consent required by the hospital. However, I understand that every effort will be made to contact me as soon as possible.

I understand that it is Light and Life’s policy to allow filming and photography of church productions and events. I understand that my specific consent has been requested for any filming or photographs that will be used for church publicity. I consent for my child to participate in this church event where they may be filmed or photographed. My consent/objection to Light and Life storing and using this material within the church organisation has been provided.
